Luanda, Angola · 15 June 2025 — Angola's largest cholera outbreak in seven years continued to expand across the country in mid-2025, affecting 18 of the nation's 21 provinces and placing enormous strain on a health system simultaneously managing other public health priorities. The outbreak, which began on 7 January 2025 in Cacuaco Municipality of Luanda Province, marked the country's first cholera outbreak since 2018.

By mid-2025, the cumulative case count had climbed steeply. As of March 23, the outbreak had recorded 8,543 cases and 329 deaths with a case fatality rate (CFR) of 3.9%. The trajectory continued upward through the second quarter, with the national CFR eventually stabilizing around 2.6% by late 2025 following intensive response efforts.

Drivers of the Outbreak

The outbreak was exacerbated by a combination of environmental, infrastructural, and social factors:

  • Rainy season: Heavy seasonal rains flooded informal settlements and overwhelmed drainage systems, contaminating water sources
  • Limited access to safe drinking water: Many communities, particularly in peri-urban informal settlements around Luanda, lacked piped water infrastructure
  • Poor sanitation: Inadequate sewage systems in densely populated informal settlements created ideal conditions for cholera transmission
  • Population movement: Internal migration and displacement facilitated the spread of Vibrio cholerae to previously unaffected provinces

A significant portion of deaths throughout the outbreak occurred in the community rather than in health facilities, highlighting challenges in health-seeking behavior and access to care. Many affected populations delayed seeking treatment until symptoms were severe, reducing the chances of successful intervention.

A Multi-Faceted Response

The Ministry of Health, supported by the World Health Organization (WHO), UNICEF, and other partners, implemented a comprehensive response plan that evolved as the outbreak expanded:

Mass Vaccination

A major reactive Oral Cholera Vaccine (OCV) campaign was executed, with over three million doses administered throughout the year. The campaign reached high-risk neighborhoods in Luanda and other affected provinces, playing a key role in reducing the case fatality rate from its March peak. The OCV campaign was complemented by targeted social mobilization to ensure high coverage in communities with varying levels of vaccine acceptance.

Infrastructure Adaptation

One of the most notable aspects of Angola's response was the adaptation of existing infrastructure originally built for polio eradication. Health authorities successfully repurposed logistics networks, mobile teams, and surveillance systems previously used for polio campaigns to address the cholera emergency. This demonstrated the value of maintaining disease-specific infrastructure as a reusable asset for broader public health response.

The Regional Cholera Context

Angola's outbreak occurred within a broader global cholera resurgence. In 2025, WHO reported responding to more than 21 public health emergencies across the African region alone, with cholera active in multiple countries including Sudan, the Democratic Republic of the Congo, and Malawi.

The global oral cholera vaccine stockpile, managed by the International Coordinating Group (ICG), faced unprecedented demand from simultaneous outbreaks. While manufacturers increased production capacity, the surge in demand forced difficult rationing decisions. Angola's allocation of over three million doses represented a significant portion of the available supply, reflecting the severity of the outbreak and the country's capacity to deliver campaigns effectively.

Challenges and Lessons

The Angola response highlighted several important lessons for cholera control in similar contexts:

  1. Community deaths matter: The high proportion of community deaths signaled that treatment-seeking behavior must be addressed alongside treatment provision. Risk communication and community engagement are as critical as clinical services.

  2. Infrastructure reuse works: The successful adaptation of polio infrastructure demonstrated that investments in one disease program can yield returns across the health system. Maintaining these networks between emergencies is a cost-effective preparedness strategy.

  3. Vaccination reduces mortality: The decline in CFR from 3.9% to 2.6% coincided with the scale-up of the OCV campaign, suggesting that vaccination — while not eliminating transmission — can reduce disease severity and improve outcomes.

  4. Religious and community leaders are essential: The mobilization of over 1,175 faith and community leaders proved instrumental in reaching populations that might otherwise have been missed by conventional health messaging.

Looking Ahead

As the outbreak continued through the second half of 2025, the focus shifted from emergency response to longer-term prevention. The WHO and partners emphasized the need for sustained investment in water and sanitation infrastructure, strengthened surveillance systems, and maintained OCV stockpiles to prevent future outbreaks.

For Angola, the outbreak served as a stark reminder that seven years of cholera-free status did not confer immunity. The conditions that enabled the 2025 outbreak — inadequate water and sanitation in informal settlements, seasonal flooding, and population movement — remain present and will continue to pose risks until addressed at the infrastructural level.
