Cape Town, South Africa — 22 January 2026 — South Africa has taken a historic step toward developing an HIV vaccine with the launch of the BRILLIANT 011 first-in-human clinical trial at the Desmond Tutu HIV Foundation (DTHF) site at Groote Schuur Hospital in Cape Town. The trial, conducted by the South African Medical Research Council (SAMRC) in partnership with DTHF and the Wits Health Consortium, is the first Phase 1 HIV vaccine study designed and led by African scientists, specifically targeting HIV strains circulating in Southern Africa.
The first participant was enrolled in late January 2026, marking a milestone in a decades-long quest for an HIV vaccine and signalling a shift in which African institutions are taking leadership roles in vaccine science rather than serving solely as research sites for external entities.
The Science Behind BRILLIANT 011
The BRILLIANT 011 study is a Phase 1 trial testing two primary vaccine components — BG505 GT1.1 and 426c.Mod.Core-C4b — administered with the SMNP adjuvant. The approach builds on the "germline targeting" strategy validated by IAVI's G002 and G003 trials in 2025, which demonstrated that the immune system can be primed to produce broadly neutralising antibodies (bNAbs) — the holy grail of HIV vaccine research.
Germline targeting works by engaging the rare naive B cells that have the potential to evolve into bNAb-producing cells. The BG505 GT1.1 immunogen is designed to recognise and activate these precursor cells, while the 426c.Mod.Core-C4b component reinforces the immune response. The SMNP adjuvant enhances the immune reaction to ensure sufficient activation.
The BRILLIANT Consortium
The trial is part of the broader BRILLIANT Consortium (BRinging Innovation to cLinical and Laboratory research to end HIV In Africa through New vaccine Technology), a pan-African initiative launched in 2024. The consortium is notably led by African women scientists, representing a deliberate effort to build indigenous research capacity and leadership across the continent.
The consortium brings together research institutions from multiple African countries, combining clinical expertise, laboratory infrastructure, and community engagement to advance HIV vaccine development tailored to African epidemiology.
Surviving Funding Cuts
The path to launch was far from straightforward. The programme faced significant risks of cancellation following the termination of certain USAID funding streams in early 2025, which led to the closure of laboratories and HIV research programmes across Africa. The original, more ambitious version of the trial was scaled back to a streamlined, "resilient" version.
The funding crisis of 2025 was described by IAVI as a "turbulent" year for HIV vaccine science, yet also a turning point. Despite laboratory closures and programme disruptions, the scientific advances — particularly the validation of germline targeting strategies — positioned the field for progress. African institutions providing "bridge funding" sustained critical research networks that would otherwise have collapsed.
Why This Matters
An HIV vaccine has eluded scientists for over four decades. The virus's extraordinary genetic diversity, its ability to integrate into the host genome, and the difficulty of eliciting broadly neutralising antibodies have made vaccine development one of the greatest challenges in biomedical research.
Previous large-scale HIV vaccine trials — including HVTN 702 (Uhambo) in South Africa, which was discontinued in 2020 for lack of efficacy, and the Mosaico trial, halted in 2023 — have ended in disappointment. The BRILLIANT 011 trial represents a new approach, applying cutting-edge immunogen design within an African-led research framework.
A Scientific Power Shift
The launch of BRILLIANT 011 has been described as signalling a scientific power shift. For decades, African countries have served as trial sites for HIV vaccine studies designed and funded by institutions in the Global North. The BRILLIANT Consortium inverts this model: African scientists are designing the studies, leading the consortia, and setting the research agenda based on the epidemiological realities of their populations.
This shift is significant not only for equity but for scientific outcomes. HIV subtype C, which drives the Southern African epidemic, has historically been underrepresented in vaccine design compared to subtype B, which predominates in North America and Western Europe. A vaccine strategy designed from the outset for the strains most relevant to the most affected populations could improve the probability of success.
